Dear Family Member/ Designated Representative,

I would like to inform you about some of the things Elmhurst Care Center is doing in order to maintain “Open Communication” with you during the COVID-19 Pandemic.

Elmhurst Care Center’s Nursing and Social Services Departments have been calling the Designated Representative/Family Members for each of our Residents in order to provide you with updates on your loved ones medical health and facility status and to answer any questions you may have. We believe that this personalized contact enables you to remain involved in your loved ones plan of care. We understand that it is very challenging and emotional to not be able to see your loved one in person at this time. We greatly look forward to the day when we can welcome you all back through the doors of Elmhurst Care Center in order to spend precious time with your loved one. Until then; we are happy to arrange for Face Time sessions and assisted phone calls in order to enable you to communicate with your loved ones. We encourage you to reach out to our Therapeutic Recreation Department to arrange for Face Time or assisted phone calls.

Elmhurst Care Center has also been updating the Facility’s website banner daily to provide up-to-date information on the amount of new COVID-19 in house positive cases in the past 24 hours as well as the number of COVID-19 positive deaths in the past 24 hours. Elmhurst Care Center is also sending out blast texts and emails including the same information daily. If you are not receiving these texts/emails please contact the Social Services Department to provide us with your cellphone number/email address. Lastly, we have a COVID-19 hotline (347-598-9091) set-up for your convenience. The hotline will ring directly to myself and I would be happy to assist you with your questions or concerns.

Elmhurst Care Center continues to prioritize your loved ones safety, comfort and well-being. We remain transparent during this pandemic and encourage you to reach out to us with any of your questions or concerns.

Elmhurst Care Center continuous to follow the guidance from the New York State and New York City Departments of Health in regards to infection control protocols. Elmhurst Care Center has appropriate PPE and screens all staff and Residents on a daily basis. Elmhurst Care Center is utilizing isolation precautions when necessary in order to minimize cross contamination. Should you have any questions or concerns please feel free to contact us.
